Sonographically Guided 14-Guage Core Needle Biopsy: Medical Audit for year 2021

Abstract

|
|
Purpose: To determine the diagnostic accuracy, pathologic underestimation rate and false negative rate of sonographically guided core needle biopsy for breast lesions.
Materials and Methods: We included 1,133 breast lesions from 948 patients who had undergone sonographically guided 14-guage core needle biopsies between January 2021 and December 2021. The sonographic BI-RADS¨Þ category of breast lesions and the pathologic results of core needle biopsies were reviewed and correlated with the final pathology result of surgery or vacuum-assisted biopsy, or the lesion's sonographic finding after more than 1 year of followup. The positive predictive value for each BI-RADS¨Þ category, sensitivity, underestimation rate and false negative rate of core needle biopsy were evaluated.
Results: Of the 1,133 core biopsy lesions, benign pathology was found in 379 (33.5%), high-risk lesions in 254 (22.4%), and malignant lesions in 500 (44.1%). The positive predictive values for each sonographic category were as follows: 0.0% (0/31) in category 3; 27.5% (192/698) in category 4a; 68.7% (90/131) in category 4b; 93.5% (145/155) in category 4c and 98.2% (109/111) in category 5. The sensitivity of core needle biopsy was 91.7% (500/545). The underestimation rate was 16.1% (41/254) for high-risk and 36.0% (49/136) for ductal carcinoma in situ. The false negative rate was 1.1% (4/379).
Conclusion: Sonography guided core needle biopsy completed during the year 2021 in our hospital was an accurate diagnostic tool for evaluating breast lesions.
